Last month we rented a car through Hertz in Florence and it was an absolute disaster. We booked an automatic Mercedes (we chose it as it was big enough to fit 4 people plus all of our luggage and seemed reasonably priced) but when we arrived at the downtown location to pick the car up we were told that firstly, they never have automatic cars and secondly, they have no large Mercedes-type cars but could give us a little Ford Fiesta or an Alfa Romeo hatchback! Neither of these cars could even fit half of our luggage - and obviously were nothing like what we ordered. This was only the beginning. They finally found a larger car (but a manual) - an Opel - but we had to go to the airport to pick it up. We asked if they could bring the car to us at the Downtown location, but they refused, so we had to get bundled into another taxi with all of our luggage and go to the airport. The car was nothing like in the class of a Merc but they insisted that the price was the same. They then even tried to then charge us an extra 160 euros for picking up the car from the airport location! Also, once they added up all the extra taxes and charges the price was nothing like what we were given on the internet - in fact it was at least double if not more - a total ripoff. We also ordered a GPS but the minute we were on the highway it stopped working and was irrepairable. Once we got to Pisa I rang Hertz there (and was hung up on at least four times but finally spoke to a manager) and was told that they didn't have any spare GPS to give us so we ended up buying one ourselves from a local shop. All in all, it was a terrible experience, the staff were unhelpful, the internet booking process completely dishonest seeing you're given nothing like what you ordered and even though we often used Hertz in other countries, we have now lost all faith in them and will never use them again.

We rented with Hertz in Italy 3 times and on each occasion have had some issues with them....each time we said we wouldn't use Hertz again, but we have! On one occasion we got a car that was completely unroadworthy, but we didn't notice until we were on the motorway - the tyres were bald and the wing mirrors were cracked so much that they were useless. We rang them from the car and had to ring them every day for almost the full two weeks of our holiday and they kept telling us they had nothing else available. We were fuming to say the least and when we got back we put in a formal complaint with photos of the car and they eventually refunded the rental cost.
We rented with them in Italy again this June, and although we had booked a four door car they gave us a two door - the person at the airport collection office was completely unhelpful and my husband ended up going back to the main office requeueing and eventually getting a replacement sorted after talking with a supervisor. The replacement car was fine but had a lot of small bangs and scratches on it which we made sure to point out and made sure they were identified on the rental agreement.
We have had no issues with them regarding the cost of the rental being different to the cost quoted.
My advice to you would be to check, and check again the state of the car they give you. If you're not happy try to get an alternative vechicle. If there are any marks on the car make sure the Hertz staff member records them on the rental agreement. Take photos of the car for your own records just in case. Check the situation with regard to fuel. We rented with a full tank and returned with a full tank - otherwise they were going to charge an astronomical amount for fuel.
I would use Hertz in Italy again if I got the best carhire rate from them - but I would be very very sure that all was right with the car before I left the car park.
